“I hereby certify that the above information is correct, and give permission for the use of photographs or videos including anyone listed above to be used in camp publicity. Proper respect for all plant life and wildlife at Camp Selah is expected. The use of tobacco, alcohol, nonprescription drugs, fireworks, or firearms are strictly prohibited. It is understood that individuals will be responsible for any damages to Camp Selah property incurred during their stay. I understand that a health officer and lifeguard will not be on duty during this event and in the case of accident or injury, I hereby assume all responsibility for all individuals listed on this form. I agree to read and comply with the campground rules and regulations posted on the premises.”
